Accountant letter on letterhead (period covered + compilation/review/audit)

Bank/credit letter if required for the aggregate you want

Surety affidavit if requesting aggregate over $2,000,000

Certificate of formation / alternate-name registration if used

Roofing applicator letters (only if requesting roofing trades)

Scan everything before you start — the portal times out

Reclassify every 24 months before the notice expires

Keep a current DPMC-701 (uncompleted work, all public + private jobs)
